There are two media consent forms: one for NYCPS use and one for use by non-profit, third parties. Contact senior field counsel or email studentprivacy@schools.nyc.gov if there are questions about which form to use.
Media Consent Form for NYCPS Use
Schools should distribute to families the Media Consent Form for NYCPS Use at the beginning of each school year and keep signed copies on file. This form gives NYCPS and members of the press at NYCPS-sponsored events permission to interview, photograph, audio record, video record, and publish a student's name, image, and likeness in print, on broadcasts, and online. The form must be signed by a parent or guardian, or by the student themself if they are 18 years or older.
Once signed, the Media Consent Form for NYCPS Use is valid for the entire school year. Schools do not need to ask parents to sign the form before each event. However, it is best practice to notify families that a school or NYCPS-sponsored event will involve the taking and publishing of photographs, audio records, video records, etc.

Third Party Media Consent Form
The Third Party Media Consent Form must be used for events and recordings (for example, filming, photo shoots, audio recordings) that are sponsored or organized by outside organizations and where these organizations will be making recordings of students. These include community based organizations that work with a school, other government agencies that may want to make recordings of students.
Please note that in such cases, forms must be completed, distributed and collected on an event-by-event basis. This form requires the entry of the name of the organization, the purpose of the recording/event, and the specific platforms where student recordings will be posted. Forms must be fully completed to be adequate.
